Eleva Fitness Rental Agreement

This agreement is between Eleva Fitness LLC and the Renter listed below. It applies to all future studio uses. Failure to comply may result in revoked studio privileges.

Rental Details

• The studio is for movement-based,parties, gatherings activities (e.g., fitness, dance, photographers,).
• Rentable in 30-min increments (1-2 hour minimum).
• Book via email, phone, or in person.

Payments

• Due at or before each rental.
• Pay via Zelle (glendagaray31@gmail.com), Venmo (@glendagaray), cash
• Late after the 5th of the month: $15 late fee.
• One-hour rental: $35 minimum.
• Extra time: $10 fee.

• Rate: $40/hour

Scheduling & Cancellations

• Weekly classes: Confirm one month in advance.
• Cancel via email with 2 weeks’ notice.
• Less than 2 weeks: full fee applies.
• Paid cancellations apply to future rentals (no refunds).

Studio Rules

• No smoking, incense, or flames.
• Stereo available.
• No storage in studio or restrooms. Use cubbies.
• Return key after each use (through the space on door)
• Lock doors after each use. Renter is responsible for loss/damage if unlocked.
• Emergency Contact: Glenda Garay (415) 654-8071. Call 911 for emergencies.

Damage & Liability

• Renter pays for any damage caused.
• All participants must sign a liability waiver.
• Renter is responsible for their guests' safety.

Clean-Up

• Turn off all lights, A/C, heater.
• Sweep and return studio to clean condition.
• Replace black mats if used.
• Clean Mirrors
• Vacuum after every class

Parking

• Plenty of parking available.

Agreement & Policy Changes

• Eleva fitness may change policies as needed. Renters will be notified.
• Signed contract may be submitted via email, in person, or left on podium.
• If any lights, speaker are left overnight there will be a $15 overnight fee.

Hold Harmless Clause Renter agrees to release and indemnify Eleva fitness LLC from any claims related to use of the premises.
